If you cannot make it to your appointment, please notify Lighthouse Counseling Services at least 24 hours in advance. Late cancelations and no-show appointments may be assessed an applicable late fee.
After scheduling your appointment, please review the Client Forms page to print the applicable forms.
Bring the completed packet with you to your first appointment. If your information has changed or is over a year old, please complete an updated packet and bring it with you to your next appointment.
